The Oblate Youth Service uses Jotform for the processing and storage of these forms.
An important note:
PHOTOGRAPH REQUIRED
Before you begin filling out these forms please have a passport style photo of yourself ready to upload. We suggest you take a photo of yourself on your mobile phone, or save an appropriate photo of yourself from your Facebook page.
Please threat the photo as you would an official photo, like a passport or drivers licence photo (i.e. facing the camera, no accessories like a hat, sunglasses etc.).
To edit your photo you we suggest you use free online apps such as Fotor https://www.fotor.com and Befunky https://www.befunky.com . These will enable you to crop and reduce the size of your photo before you upload it. Please crop it to a square shape and reduce the file size to less than 500kb.
